Something’s missing and something worth noted

What I've observed for the past few months.
It seems that we lack of (or I shall put it this way, have
insufficient) communication. What I noticed in the first place is
that on our intranet(?)/automation system, it seems that some
functions are designed for one person, and everyone seems to have
his/her own “tool”, which cause redundancy and reduce efficiency.
And the lack of communication between department also cause a
problem which I noticed becuz I'm placed in the middle, and whenever
one ask me to make some changes, the other will jump up later saying
that something's been changed and doesn't work for him/her.
I think we need to build a more efficient and more open(?)
communication model to reduce misunderstanding and unnecessary
modification and duplication.
I know I may not see the situation clearly. Afterall, I'm just
a rookie in the work field. Above is just my HUMBLE opinion.
As for myself, there are some things I have to remind myself
constantly:
My attitude towards colleaques and superiors. Dont act arrogantly
like I know everything and I'm always right (which clearly I dont
and I'm not.) Be humble, polite, and less offensive/defensive.
Make a space in mind for other possibilities. Don't just respect
the poeple I like.
